It is important to bring the fully completed and signed patient packet to your visit. If you are unable to complete online please allow 30-45 minutes prior to your appointment to complete the patient packet. It is equally important to have with you at the time of the appointment your insurance card.
Your films/CD need to be brought with you at the time of your visit or your appointment may have to be rescheduled. Bringing just the reports, test results, and/or records are not accepted. You are being seen at this office by a specialist in neurosurgery. The doctor requires the films/CD to view your medical condition, if any, in order to render a medical opinion.
If you are unable to keep your appointment, please give us at least 24 hours notice or a no show fee may apply. This fee is not billable to your insurance company.
Your insurance copay, deductible, coinsurance will be collected from you at the same time we collect your patient packet, films/CD, reports, test results, and/or records.
Returned checks are subject to a handling fee of $25.00.
